As a Principal Care Coordinator (Healthcare), you will be responsible for the following duties:

  • Lead the team in providing support services and programs to patients and their families, ensuring optimal health outcomes and enhancing home-based coping mechanisms.
  • Lead the team in conducting follow-up after discharge (via phone or home visits) to ensure patients and caregivers can cope effectively, coordinating community services, and providing education on caregiver responsibilities, including end-of-life care planning.
  • Supervise and manage the input, maintenance, and analysis of data and data systems.
  • Collaborate with partners in Acute Care and ILTC settings to strengthen ongoing support for patients and families post-discharge from community hospitals, aiming for optimal health outcomes and reducing unnecessary hospital readmissions.
  • Develop and implement programs and services to meet the needs of patients and caregivers in the community.
  • Manage staff performance and ensure the clinical competence of the team.
  • Plan and oversee the professional development and training requirements of the team.
  • Evaluate and refine operational processes with internal stakeholders and external organizations to ensure quality of care and operational efficiency.
  • Participate in or lead hospital initiatives (such as quality improvement projects, LEAN projects, or research projects) to enhance the quality of clinical care.

Requirements:

  • Possesses a professional qualification in Nursing, Social Work, Allied Health, or other relevant clinical disciplines 
  • Brings at least 8 years of experience as a Care Coordinator or Case Manager in either acute or community healthcare environments.
  • Demonstrates a minimum of 3 years of experience in team leadership roles.
