As a Senior / Pharmacist, you will be responsible for the following duties:

  • Deliver clinical pharmacy services independently or as part of a multidisciplinary healthcare team to enhance patient wellness and optimize medication outcomes across various diseases.
  • Ensure pharmaceutical care meets defined standards, promoting the safe and effective use of medicines.
  • Provide education and training to trainees, pharmacy students, support staff, and the public.
  • Mentor and supervise trainees, pharmacy students, and support staff in their professional development.
  • Manage clinical pharmacy services within assigned patient care areas.
  • Collaborate with healthcare teams to optimize medication therapies and improve patient outcomes.
  • Assess patient-specific medical issues, recommend and manage pharmacotherapy, counsel patients, dispense medications, and monitor drug therapy adjustments.
  • Identify, resolve, and prevent medication-related problems following departmental policies.
  • Encourage adherence to prescribing guidelines and drug use protocols.
  • Offer professional, evidence-based drug information to patients, students, and healthcare providers.
  • Work with patients, caregivers, and providers to ensure safe, cost-effective, and timely medication use, supporting continuity of care.
  • Communicate with patients and caregivers individually or in groups to ensure understanding and adherence to prescribed treatments.
  • Document clinical activities as required by competency frameworks and department policies.
  • Participate in clinical practice and other relevant departmental committees.
  • Develop and deliver educational programs for trainees, pharmacy students, support staff, and the community.
  • Contribute to the creation and dissemination of patient education materials.
  • Guide and supervise the professional training and development of pharmacy trainees and staff.
  • Support the development and implementation of pharmacy professional standards, competency frameworks, and assessments for pre-registration pharmacists.
  • Engage in ongoing professional development activities.
  • Assist in reviewing and improving services, policies, and processes to enhance medication safety and quality at the institutional level.
  • Participate in or oversee pharmacy activities related to clinical work.
  • Perform any other duties as assigned.

Requirements: 

  • Bachelor of Science in Pharmacy from an accredited institution. 
  • Registered with the Singapore Pharmacy Council (SPC).
  • Postgraduate qualifications relevant to hospital pharmacy; hospital pharmacy experience preferred but not mandatory.
  • Strong communication, supervisory, and leadership skills with at least 12 months of pharmacy operations management experience.
  • Ability to collaborate effectively with pharmacy, medical, nursing, and other healthcare personnel.
