As a Care Coordinator, you will be responsible for the following duties:
- Manage billing-related administrative tasks, including accurate data entry in the SAP system.
- Process subvention and subsidy applications through relevant platforms, such as:
- ILTC Portal for Intermediate and Long-Term Care processes
- SMF Forms for Seniors’ Mobility and Enabling Fund applications
- Coordinate admissions and schedule visits for Home Care Services clients.
- Perform general administrative duties, such as filing, maintaining records, and managing documentation.
- Handle phone and email enquiries related to healthcare administrative and financial matters, ensuring clear and professional communication with clients and internal teams.
Requirements:
- Possess a diploma in science-related subjects
