Job Description

• Office hours; 5.5days work
• Central location with close proximity to MRT
• Senior position available

As a Care Manager, you will be responsible for the following duties:

• Performing triage and measurement of parameters
• Documentation of patient interactions in medical records
• Reviewing clinical data from lab tests and glucose monitoring.
• Provide advice and educating patients and caregivers on medications, glucose monitoring, prevention and monitoring of complications
• Supervision and guidance of junior nurses
• Perform diabetic foot screening duties
• Patient Counselling duties

Requirements:

• Min. Diploma in Nursing
• Candidate with Specialist Diploma will be a plus
