As a Case Management Executive, you will be responsible for the following duties:

  • Working with Medical Social Worker in assessing, evaluating, planning and coordinating resources and services for residents.
  • Collaborating with multidisciplinary health service professionals to develop and implement care programs that ensure timely treatment and recovery for residents. 
  • Monitor the bed occupancy rate and ensure that it aligns with the admission Key Performance Indicators (KPIs) on a monthly basis. 
  • Analyzing trends and report any discrepancies or areas of concern to ensure optimal bed utilization. 
  • Keep accurate and up-to=date records of residents' data and information. 
  • Apply relevant financial schemes for residents and monitor the associated pay-outs on a monthly basis.
  • Ensure that financial assistance is appropriately allocated and processed according to established guidelines.  
  • Monitoring the renewal status of residents' Household Means-Test to ensure it is updated before it's expiration. 
  • Coordinate with relevant parties to facilitate timely renewals.
  • Prepare and compiling Medifund reports and documents required for submission to the Medifund Committee. 
  • Ensure all reports are accurate, complete and submitted within the required timelines. 
  • Addressing and resolve queries and concerns from residents and their next-of-kin in a timely and professional manner. 
  • Provide support and information to residents and families regarding services, policies and care plans. 
  • Assist with any additional case management tasks as assigned, ensuring all aspects of resident care and services are manged effectively.

Requirements:

  • Diploma in Nursing, Social Work or other related disciplines.
  • Minimum 1 year of experience in community healthcare settings in Singapore.
  • Good knowledge of the geriatrics and community services.
