As a Social Worker, you will be responsible for the following duties:
- Conduct comprehensive assessments of clients' needs, strengths, and risks.
- Develop and implement individualized care plans in collaboration with clients and their families.
- Provide ongoing case management, including regular follow-up, monitoring, and evaluation of client progress.
- Coordinate and refer clients to appropriate internal and external resources and support services.
- Provide counseling and emotional support to individuals, couples, and families facing various challenges.
- Facilitate group work and workshops to address specific client needs and promote personal growth.
- Assist clients in developing coping strategies, problem-solving skills, and resilience.
- Conduct crisis intervention and risk assessment when necessary.
- Maintain accurate and up-to-date case records, progress notes, and statistical data.
- Prepare reports and assessments for internal and external stakeholders as required.
- Adhere to confidentiality and data protection policies and procedures.
Requirements:
- Bachelor's Degree in Social Work
