As a Patient Engagement Executive, you will be responsible for the following duties:
- Engage patients to encourage participation in PCN programmes, including Healthier SG, Chronic Disease Management, diabetic screenings, nurse counselling, and community care
- Coordinate with partner and affiliate clinics to refer eligible patients for ancillary services like Diabetic Foot Screening, Diabetic Retinal Photography, Nurse Counselling, and Spirometry
- Conduct outreach calls and follow-ups to schedule appointments, send reminders, gather feedback, and provide programme information
- Support clinics in identifying eligible patients, tracking defaulters, and monitoring progress
- Assist in referring identified patients to community and social partners such as Active Aging Centres, Day Care, and Home Care services
- Provide logistical support for nurse deployment to partner and affiliate clinics, ensuring smooth coordination
- Maintain accurate patient records and update internal tracking systems
- Handle patient queries and concerns professionally, empathetically, and confidentially
- Assist in preparing reports, tracking KPIs, and ensuring compliance with documentation requirements
- Perform other tasks as assigned by the Team Lead
Requirements:
- Diploma in relevant field with familiarity in clinic operations; experience in call center, customer service, hospitality, or healthcare preferred
- Strong team player with good interpersonal and communication skills; tech-savvy and proficient in Microsoft Office; knowledge of clinic management systems a plus
- Positive, adaptable attitude with the ability to handle changing situations effectively
